Vietnam: Compilation of the technical requirements for linear accelerator machines used in radiation therapy

On December 5, 2017, the Ministry of Science and Technology of Vietnam issued Circular 15/2017/TT-BKHCN regarding the National Technical Regulation for Linear Accelerators used in Radiotherapy.

According to Circular 15, linear accelerators used in radiotherapy must meet the following requirements:

No. Inspection Content Technical Requirements
I External Inspection - The radiotherapy linear accelerator to be inspected must be in working condition.

- The radiotherapy linear accelerator and control console must be clean, free of rust, cracks, or deformation.
II Technical Inspection  
1 Control Console Inspection - The switches on the control console must function properly and move smoothly;

- Indicator lights on the control console must accurately reflect the operating status of the linear accelerator.
2 Linear Accelerator Operation Inspection - The linear accelerator arm must rotate smoothly around its axis and be able to stop at any position.

- The beam collimator of the linear accelerator must rotate easily around its axis and stop at any position.

- MLC leaves or jaws can move easily under control to create the required radiation field.

- The light field size indicator must be clearly visible.

- Mechanical distance measurement systems, optical indicators, and laser systems must be clearly visible.

- The emergency stop system allows stopping irradiation from the control console, and resuming irradiation after stopping can only be done from the control console.

- The interlock system operates stably.

A Mechanical System Accuracy Inspection  
1 Accuracy of Optical Indicator Ruler ± 2 mm
2 Accuracy of Rotation Angle Display in the Beam Collimator System, Arm, and Treatment Table ± 1°
3 Congruence of Light Field and Radiation Field Size ± 3 mm for field sizes ≤ (200 mm x 200 mm);

± 5 mm for field sizes > (200 mm x 200 mm)

± 2 mm deviation of the radiation field center from the crosshair center
4 Laser Beam Accuracy at the Isocenter ± 2 mm
5 Accuracy in the Treatment Table Movement ± 2 mm
B Photon Beam Radiation Dose Inspection  
6 Photon Beam Quality (TPR20/10) ± 2% compared to the TPR20/10 or PDD20/10 value at the time of commissioning
7 Photon Beam Output Dose Calibration 1 cGy/MU ± 2%
8 Output Dose Factor according to Field Size ± 2% compared to the output dose factor value at the time of commissioning
9 Field Flatness (F) and Symmetry (S) ± 3%
10 Transmission Factor of Wedge (Physical Wedge) ± 2% compared to the transmission factor value at the time of commissioning
C Electron Beam Radiation Dose Inspection  
11 Electron Beam Quality (R50) ± 2 mm compared to the R50 value at the time of commissioning
12 Electron Beam Output Dose Calibration 1 cGy/MU ± 2%
13 Output Dose Factor for Electron Beam Applicators ± 2% compared to the output dose factor value at the time of commissioning
14 Field Flatness (F) and Symmetry (S) for Electron Beam F ≤ ± 6% for energies less than 10 MeV; F ≤ ± 4% for energies greater than 10 MeV

D Multi-Leaf Collimator (MLC) Inspection  
15 Accuracy of MLC Leaf Positioning at Isocenter ± 1 mm compared to the MLC coordinate value established by software
16 Congruence of Radiation Fields Created by MLC When Rotating the Gantry (Gantry Spoke Shot) All intersection points lie within a circle with a radius of 1 mm
17 Congruence of MLC-Created Radiation and Light Field Sizes ± 2 mm
18 Congruence of Radiation Fields When Rotating the MLC (MLC Spoke Shot) All intersection points lie within a circle with a radius of 1 mm
19 Radiation Leakage through Leaf Gaps and Body (Interleaf/Intraleaf Leakage), Leakage through Leaf Tips ≤ 2% (through leaf bodies), ≤ 2.5% (through leaf gaps), and all values of radiation leakage through leaf bodies, gaps, and tips must be incorporated into the radiotherapy planning simulation software
