The medical incident report includes voluntary reporting and mandatory reporting. Depending on the situation with varying degrees of injury, the corresponding reporting form is applied.
To be specific, voluntary reporting is applied for situations where the degree of injury to the patient's health and life is:
- Not yet occurred (NC0);- Light injury (NC1 - injury is self-healing or does not require treatment intervention);- Moderate injury (NC2 - injury requiring treatment intervention, prolonging hospital stay, affecting long-term function).
Mandatory reporting is applied for situations where the degree of injury to the patient's health and life is severe (NC3 - injury requiring emergency intervention or major treatment intervention, causing permanent loss of function or resulting in death).
