This content is specifically guided by Circular 01/2016/TT-BYT issued by the Ministry of Health on January 09, 2016, which stipulates the prescription of traditional medicine and the combination of traditional medicine with modern medicine.
Circular 01 stipulates that in the event of a prescription amendment, the prescriber must sign, clearly state their full name, and indicate the date beside the amended content;
Additionally, Circular 01 sets forth that when prescribing medication, the following conditions must be met:
- The writing must be clear and precise, and all sections printed in the prescription or the patient's medical records or the treatment sheet in the medical record must be fully filled out;- The address of the patient's residence must be accurately recorded according to administrative regulations about localities: House number, street, residential group or hamlet, village, commune, ward, commune-level town;- For children under 72 months of age, the number of months of age must be recorded along with the name of the child's father or mother;- Strike through the empty portion of the paper from below the prescription content to above the prescriber’s signature; sign, and clearly state the full name of the prescriber.
